Attendance Agreement Form

If it is determined that a student is eligible for the accommodation of flexibility with attendance, the student and instructor must work together to complete the following Attendance Agreement Form.

Prior to the meeting, instructors must review the Questions to be Considered by the Instructor.  

The student and instructor will then work together to create an accommodation plan. The Attendance Agreement Form will be completed by the instructor, signed, and then sent to the student to be completed and signed. The instructor must be the first one to open the form in order for the form to work correctly.

The Attendance Agreement Form should be completed and signed before the last day to add a class has passed. This accommodation is not in effect until the form has been submitted. This form is not a determination of whether a student is eligible to receive an accommodation, but rather how the accommodation will be implemented in an individual course.

Questions to be Considered by the Instructor

  1. How much interaction is there between instructor and students during class meetings (e.g., discussion, interactive experiences, problem solving)? Is this the case for all class sessions, most sessions, some sessions, or none?
  2. How much interaction is there among students during class sessions (e.g., peer review, group discussion)? Is this the case for all class sessions, most sessions, some sessions, or none?
  3. Would missing class prohibit students from meeting course learning objectives (e.g., lab experiments, performance work)? Is this the case for all class sessions, most sessions, some sessions, or none? 
  4. To what degree does a student’s absence or variable attendance constitute a significant loss of the educational experience for the student and/or classmates (e.g., group or paired projects, performances) and/or cause a disturbance to course instruction? Significant, moderate, minimal, or none?
  5. Are there alternative ways the student could catch up on missed material (e.g., attend other lab/class sections, alternate assignment)?
  6. How is attendance calculated in the final grade according to the course description and syllabus?
  7. How are absences weighed for reasons such as athletic travel or non-disability related illness?
