Deadline Agreement Form

If it is determined that a student is eligible for the accommodation of flexibility with deadlines, the student and instructor must work together to complete the following Deadline Agreement Form. 

Prior to the meeting, instructors must review the Questions to be Considered by the Instructor.  

The student and instructor will then work together to create an accommodation plan. The Deadline Agreement Form will be completed by the instructor, signed, and then sent to the student to be completed and signed. The instructor must be the first one to open the form in order for the form to work correctly.

The Deadline Agreement Form should be completed and signed before the last day to add a class has passed. The accommodation is not in effect until the form has been submitted. This form is not a determination of whether a student is eligible to receive an accommodation, but rather how the accommodation will be implemented in an individual course.

Questions to be Considered by the Instructor

  1. What are the essential course requirements?
  2. What does the syllabus indicate regarding late work and completion deadlines?
  3. Is the material being learned in the class sequential? Does each assignment or each week’s material build on the material learned in previous assignments or week? Is this the case for all assignments, most assignments, some assignments, or none?
  4. Does the fundamental nature of the course rely upon timely completion of assignments as an essential method of learning (e.g., assignments must be submitted before an exam, class discussion, group project)? Is this the case for all assignments, most assignments, some assignments, or none?
  5. To what degree does the failure to submit timely completion of assignments constitute a significant loss of the education experience for the student or their classmates? Significant, moderate, minimal, or none?
