BMI: Secondary Accident and Injury Insurance Plan

Bates College provides a student accident insurance plan through Bob McCloskey Insurance (BMI) for the 2023-2024 school year. Enrolled students are covered for Accident Medical Expense Benefits and Accidental Death and Dismemberment Benefits subject to the terms, conditions, limitations, and exclusions of the Policy. It covers certain medical expenses for the treatment of injuries that are the direct and independent result of a Covered Accident during the policy period.

Frequently Asked Questions

All enrolled students are eligible for coverage under the BMI Student Accident Insurance Plan.

This plan provides coverage for “Covered Accidents.” A covered accident is a “sudden, unforeseeable external event that results, directly and independently of all other causes, in an injury or loss and meets all of the following conditions: 1) occurs while the Covered Person is insured under this Policy; 2) is not contributed to by disease, sickness, or mental or bodily infirmity; and 3) is not otherwise excluded under the terms of this Policy.”

This plan covers eligible expenses for covered accidents up to $2,500. Eligible medical expenses must be incurred within the 104-week Benefit Period; with the first eligible expense incurred within 180 days of the Covered Accident. The claim form needs to be submitted within 90 days from the date of the accident. Benefits are payable for eligible expenses that are in excess of benefits paid by any other health care plan.

In order to use the BMI insurance plan there are several steps you must:

  • Complete the claim form in its entirety and submit to BMI Benefits, within 90 days from the date of the accident.
  • Bring the BMI insurance card to all medical providers where you access care for the covered accident, or contact all medical providers where treatment was received and provide BMI’s billing information so they can bill BMI directly.
  • Or, submit all itemized bills and primary insurance E.O.Bs to BMI Benefits for processing of outstanding balances due to a Covered Accident.
  • Submit these to BMI at:

BMI Benefits, LLC.

If you are accessing medical care for an injury or accident, you will need to complete the Wellfleet “Accident Form” in order for your claims to be processed. You will need to fill out this form in addition to the BMI claim form as detailed above. In order to do this, log into your Wellfleet account by selecting the Student Health Insurance Portal in the Garnet Gateway.

Once you log into Wellfleet, locate the tab across the top that says “Claims.” Click on this tab, and locate the “Accident Form.” Select this “Accident Form.” This will bring you to another page where you will have to enter information about your recent services. Wellfleet will need this form in order to process your claims.

As stated above, all claims will need to be processed by primary insurance companies before being processed by BMI.
