Request Form — Teaching Assistant Card

The faculty member is responsible for all materials borrowed by the T.A. The student is restricted from using the card for personal use. Courtesy notices, and overdue notices are sent to the faculty member’s email account.

All fields with a red asterisk are required.

  • Each card is tied to a specific student and is only valid for the time that student is working with you - usually a semester or two.
  • This field is for validation purposes and should be left unchanged.
