Hello Bates Class of 2028 families – 

We write with an important reminder for you and your student. Health forms for entering students are due Monday, July 1st. This includes the Health History, Consent for Medical Treatment, and TB Screening Forms and the Vaccination Form.

Frequently asked questions and links to the forms can be found on the Bates Health Services website. Students under the age of 18 can access the forms here. All students who need to submit forms received instructions to their Bates email address on Friday, June 21st. If your student did not receive instructions to their Bates email address, no further action is needed at this time.

All entering students must fill out all of the first-year forms in Garnet Gateway prior to matriculating to Bates in the fall. A full schedule of open and due dates for these forms can be found here.
